Successful Community Care Scheme To Be Expanded
The successful team at the Vale Community Resource Service (VCRS), which helps move people out of hospitals and back into their own homes, is to be expanded in order to enable it to continue to provide a seven-day service to support hospital discharges.
VCRS, which is largely funded by the Vale of Glamorgan Council and Cardiff and Vale UHB, is to be expanded following an investment by Welsh Government.
The scheme provides support for people when they first come home from hospital and, once in place, are able to work to reduce the need for any future admission. The team use person-centred active support to help people to do as much as they can for themselves – this means motivating the person by doing things ‘with’ them and not ‘for’ them, and then gradually stepping back as they regain their confidence.
Cllr Bronwen Brooks, Cabinet Member for Housing, Social Care and Health, said: “The Vale Community Resource Service is an excellent example of public sector organisations working in partnership to improve the lives of Vale residents. They have achieved great things since they were established and will now be able to do even more with a larger team.”
The team currently includes an occupational therapist, physiotherapists, dietitians, speech and language therapists, nurses, reablement coordinators, reablement support workers, rehabilitation technicians, falls technicians and therapy assistants. VCRS is now looking to employ a number of Reablement Support Workers to enable them to provide support to even more people in the Vale.
An open day is being held at Barry Hospital on 14 September 2016 where people can find out more about how the team works. Application forms for the new posts will be available on the day.
