New Social Services Online Self-Assessment Form Is Up And Running
Anglesey Social Services free online social care and health assessment was recently launched on the council’s website to help support and maintain people’s independence.
The online form has been designed to assess an individual’s needs, be it from needing information and advice to general community support.
Anglesey’s Head of Adult Services, Alwyn Jones said, “This new and innovative way for individuals or their carers to see what their health and social needs are is a great alternative to telephoning, visiting or emailing Social Services.”
“The aim of the new scheme is to further assist Social Services to support people’s independence, and I would encourage anyone who thinks they may need information or advice, signposting to general community support, and where appropriate, access into health and social care community services to complete the online form today.”
The online referral form is quick and simple and provides individuals with an additional means of contact with Social Services. The form itself should only take between ten and 15 minutes to complete, but can be completed at your own leisure once you have registered. Once completed and sent you should expect a response within three working days.
If you are completing the referral form on behalf of another person, you will need that person’s consent to do so and tell us as much as possible about the person’s situation in order to help us understand their needs.
